'the Psychologist': the PCQ Psychologist providing a service to you
Psychological Service As part of providing a psychological service (i.e. psychological assessment, counselling, injury rehabilitation management) to you, Psychology Centre (Qld) will need to collect and record personal information from you that is relevant to your current situation. This information will be a necessary part of the services provided. You do not have to give all your personal information, but if you don’t, this may mean the psychological service may not be able to be provided.
Purpose of collecting and holding information Information is gathered as part of PCQ services and is seen only by the psychologist. The information is retained in order to document what happens during consultations (including phone consultations/discussions), non-consultation phone calls and emails, and enables the psychologist to provide a relevant and informed psychological service.
Information is also collected for the purpose of an assessment.
Access to Client Information Psychology Centre is committed to maintaining client privacy and confidentiality. All records are securely stored and only your psychologist at Psychology Centre has access to your file. Discarded material with any personal information, is destroyed with an office shredder.
At any stage, you as a client are entitled to access the information about you kept on file, unless relevant legislation states otherwise.
Use and Disclosure Psychology Centre will use and disclose your information for purposes such as:
Providing a report about your psychological health, progress and/or work capacity to the person or organisation that referred you to PCQ (this may be your GP, employer, an insurer, law firm or other body);
Account keeping and billing purposes to a referring company (usually name and case/policy number only)
Referral to another health care provider;
Subpoenaed by a court or authorised by law;
Where failure to disclose the information would place you or another person at serious and imminent risk;
Discussing the material with another professional for their input/recommendations.
The consent provided at the end of this document, allows permission to collect and release only relevant information from any parties described above.
